One study, based on the North American Antiepileptic Drug Pregnancy Registry, found the relative risk of oral clefts in babies exposed to Topamax® before birth was 21.3 as compared to babies whose mothers were untreated during pregnancy.   The risk from Topamax® was also found to be 5 to 7 times higher than that from other anti-seizure drugs.

Cleft lip and cleft palate are birth defects that occur when the lip or palate do not fuse together during the first trimester of pregnancy.  These defects can affect the appearance of the face, lead to problems with feeding and speech, and cause ear infections.  Usually, surgery is required to close the lip or palate.

After the March 2011 FDA Safety Communication, Topamax® is being placed in Pregnancy Category D, since there is evidence of human fetal risk.  It is advised that Pregnancy Class D drugs such as Topamax® only be used in a life threatening situation when safer drugs are consider ineffective or cannot be used.

The FDA recommends that before starting Topamax® or the generic equivalent women of childbearing age should discuss treatment options with their health care provider.
